Here is what you need to list on the label for a preservative: Propylene Glycol (and) Diazolidinyl Urea (and) Iodopropynyl Butylcarbamate
You don't use the "brand" name.
If you are going to use e-wax, I would suggest either e-wax NF or Polawax. Generic e-wax has a different INCI and might not work in your formula (or as well).
As far as labels, have you checked out onlinelabels.com or rippedsheets.com ? I believe at least one of them can print labels for you.
I use 2 x 4 labels for bullet bottles.

Thanks! The ewax that I was thinking of using is Crafters Choiceâ„¢ Emulsifying Wax NF - Soft & Silkyfrom WSP. Is that allright? Do I need to list the exact ingredients on that also? I will look into those sites.

Does goat milk need to be pastuerized before I make lotion with it? I pastuerized the milk I used the other day. I just hate the wmell of rotten milk and if I'm not careful it can get stinky on my milking equipment etc.

Milk is notoriously hard to preserve due to milk proteins, which germs and bacteria just love. That's one of the reasons why I don't do it.
If you decide to go this route, I would strongly suggest you get you lotions professionally tested.
In some instances, you would need more than one preservative for their combined efficacy.

PS. Yes, it has to be pasteurized.

I don't take milk handling light heartedly. I think thats a great idea to test it. Should I also sterilize the containers? What about storing it in the freezer before selling it? If I were to combine preservatives would it still account for 1% of the recipe?

If you have sterilization equipment; go for it. For the most of us, the best we can do is sanitize.
I wouldn't store the finished product in the freezer. It could change the feel, the texture or at worst, separate, when it thaws back out.

The total percentage of both preservatives would depend on which you are adding. Some have different HLB values and might not be compatible togather.
