There is a rule and it is that you must list your physical address. Company name, full address, no po box. It is on the FDA website if you want to read it for yourself.
We are trying to get that changed, but as of now, this is a must unless your company is in the yellow pages, then you can list just your company name, city, state and zip code (without a street address).

I wonder if that includes having a website with a physical address. Both of the soap makers I purchased bars from at the last craft show I went to had their websites listed but not a physical address. There was an address on the site. I haven't had time to look it up myself, just thought it was intereresting.

According to the FDA the label must carry the physical address. Websites do not require an address, just a way to contact you, which email covers.

IIRC, you don't need to have an address if you're listed in the phonebook under your company name.

Yes, that was stated above. If you are listed in the yellow pages you can just list your city, state and zip.

Is it specifically in the yellow pages portion of the phone book or do the white pages count to?

If I recall correctly, last I looked, it was specifically the yellow pages, but that was a few years back so you'd have to search again to verify. I would think that as long as someone could call information, give your company name, and get your physical address and phone number you'd be okay. The idea is that they can locate you. Another reason I am behind allowing po boxes, I don't want every wackadoodle to have my home address and my shop is right behind my house. same address!
