When my girls were babies, we discovered that one of the absolutely best things to do for diaper rash was give them a bath with baking soda sprinkled in the water. That was all it took, and their rash would be gone within 24 hours! For severe cases we used Triple Paste - which, let me tell ya, is the thickest toughest paste. But really, the most important thing that a diaper cream does is create a water barrier. Anything that seals the rash away from wet diaper will do the job just fine.
I didn't use any kind of soap on my kids until they were around 12 months old. Just some bubble bath in the water, and they were good to go.
