Simply what questions should I be asking? any help with links and such would be helpful. I am not good with words or questions and I am totally stumped.

Are we talking about samples for something? If so, what kind of product?

What is the questionnaire for? What information are you hoping to obtain?

its mainly for soap and lotions. I'm just wanting to know if I'm making a good product. I know its safe but not sure how it feels and such. I know am not making any sense. but just basic questions on the product mainly for feedback. but I'm stumped at what kinds of questions to ask about a product.

For soap, you could have a rating scale, like 1-5 or 1-10 and have them rate bubbles, lather, strength of the fragrance, size of the soap, color, packaging. Then have them tell you what they enjoyed about it & why. Then what they didn't like about it & why.

But really it comes down to what would you like to know about your soaps? If you are trying out someone else's soaps/lotions, what's the first things that pop into your mind?

Agree with Genny. I have done questionnaires for things at work and I find I get the most responses when they are quick and easy, like giving a numbered rating scale they can circle or multiple choice. At the end I leave a space for comments.

To get answers for specific questions that you are interested in using to tweak your product, I suggest giving them multiple choice type answers so you know more than just "it was good" or "it was bad".

1. Rate the strength of the fragrance, circle one: a)Too strong b)Just Right c)Too Weak
Comments:

2. Rate how the lotion felt when you rubbed it on your skin, circle one: a) Too greasy b) Moisturizing and soft c) Drying
Comments:

etc, etc.

Most people won't take the time to write comments, so you get more info this way. I also suggest writing the name of the product they are giving feedback on and providing an anonymous way to get the feedback to you, like having it on a stamped postcard or even doing it online. Survey Monkey is pretty easy and you can use the basic format free. Good luck!
